The Center for Healthy Aging provides healthcare services focused exclusively on serving the older population of the Appalachian Highlands. This new clinic is designed to coordinate services most relevant to older adults and seniors, with the mission of improving health and wellness for the aging through a multidisciplinary team approach.

We focus exclusively on serving patients 65 years and older with accessible, high-quality, cost-effective care.

The Center’s specialized staff is led by geriatric doctors and supported by best-in-class patient navigation and care coordination systems, which will result in better outcomes, a reduction in avoidable readmissions and better overall independence and quality of life for our older patients.

Clinic hours

We’re open Monday through Friday, 8 a.m. to 5 p.m.

Same-day and next-day appointments are available for established patients.

Specialized healthcare for aging adults

The Center for Healthy Aging offers primary care treatment and services and geriatric services for aging patients throughout Sullivan County and our surrounding communities. Just as children see a pediatric specialist to meet their specific health needs as they age, older people can benefit from seeing a geriatric specialist to meet their specific health needs as they age.

The Center’s geriatric internal medicine physicians have specialized knowledge of the aging process and are skilled in the diagnostic, therapeutic, preventive and rehabilitative care of the elderly. We see patients aged 65 and older.

Experienced geriatric internal medicine

The Center’s age-friendly primary care team provides top-quality services, treatments and specialist referrals to match the unique needs of our older adult and senior patients. We offer treatment and expert care for:

• Acute conditions or illnesses that happened recently and require immediate care, such as a sprain, minor wound or burn, ear infection, strep throat or the flu
• Chronic conditions or illnesses that need ongoing care, such as diabetes, heart disease, bladder control problems or dementia

We’ll work with you to develop an individualized treatment and care plan to help you manage your condition or symptoms.

By having all geriatric services available at one location, the Center will make it simple for patients to gain access to the care they need. Referral services will include:

• Cardiology
• Audiology
• Physiatry
• Physical therapy
• Palliative care
• Clinical pharmacy
• Neurology assistance with dementia and gait/imbalance
• In-home care with multi-disciplinary home visits team
• Advanced frailty care programs
• Incontinence/urogynecology clinic
• Nutrition services

A coordinated approach to healthy aging

Our coordination of care with local pharmacies will make it easier than ever to bundle medications and to sign up for at-home delivery for your convenience.

The Center also coordinates with medical equipment companies to ensure advanced medical supply needs are met, as well as coordinating with diabetic services and nutritionists for meal plans.

Our comprehensive care management will not only focus on a patient’s physical needs, but also their social and emotional needs. These services include counseling, depression screening, assisted living, chaplain services and group activities.

Please note that we are not a long-term care facility or nursing home.
