MyChart is a completely free online service that gives you secure online access to your medical records, test results, appointments, and more. All Ballad Health Medical Associates clinics and Ballad Health facilities use MyChart, so if you are a patient at a BHMA or Ballad Health location, you can use MyChart to manage your care online.
Patients who are 18 years of age or older treated within the Ballad Health system, including clinics and hospitals, may enroll in MyChart.
Patients aged 17 and younger must be attached to a parent or guardian's MyChart account by completing the MyChart Proxy Request and Authorization Form [PDF, 146 KB].
This form can be completed in person at a Ballad Health Medical Associates provider's office or nearest Urgent Care. A valid photo ID or any necessary legal paperwork is also required.
MyChart Proxy forms can also be completed and e-mailed to mychartcentral@balladhealth.org, along with a valid photo ID or any necessary legal paperwork.
Please note: For patients between 12- and 17-years old, there will be limited access due to regulations set by the states of Tennessee and Virginia unless teen proxy is appointed.
Once you log in through your computer or the Ballad Health app, look for Sign Up Now on the login page.
If you have an activation code:
Choose the Activation Code option. Enter your code, the last four digits of your Social Security number and your date of birth. Click Next to continue with the enrollment process and create your username and password.
If you do not have an activation code:
Choose the No Activation Code option. Enter your name, address, date of birth, Social Security number, sex and email address. Click Submit to continue with the enrollment process and create your username and password. Or you may call our Ballad Health MyChart Technical Support Team at 1-866-517-5873 Monday through Friday, between 8 a.m. and 5 p.m. for an instant activation code.

Proxy access allows parents, legal guardians and caregivers to manage a patient's MyChart account and assist with their medical information.
Proxy types: Child, Teen, Diminished Capacity, Power of Attorney and Adult.

Due to the sensitive nature of medical information, each adult must establish their own MyChart account. However, you may authorize access to either of your accounts by completing and returning your proxy forms.
MyChart provides a "snapshot" of your medical history at Ballad Health. If you need information not available in your MyChart account, you may request records online via your MyChart account by choosing Menu > My Record > Document Center > Requested Records. You cannot change your MyChart username once it has been created.
Two-step verification is a security feature in MyChart designed to help Ballad Health keep your patient information as secure as possible. If you desire, you can turn this feature off by logging into your MyChart account. Select "My Account" and then "Security Settings." If you require assistance, you can call the MyChart Technical Support team at 1-866-517-5873.
New passwords must be at least 10 characters and contain at least one capital letter, one number and one special symbol.
Once you reset a password, you cannot use a previous one.
